In the past two weeks I've had a hanger steak cooked medium-well (I requested medium-rare) and a burger booked well-done (I requested medium-rare). Both were in Virginia. I didn't send either dish back (probably should have)...the real lesson is probably to stop eating out in Virginia (except that the medium-well steak was at a generally good restaurant).
